Proposed DSM-5 Criteria May Unfairly Label Physical Conditions as ...

Recently in the IBS and chronic illness community, several professionals and self-advocates have begun expressing concern about proposed changes in the Diagnostic and Statistical Manual for Mental Disorders, commonly known as the DSM. This is a lengthy reference book published by the American Psychological Association as a guide for mental health professionals in diagnosing their clients or patients according to standard criteria. The DSM is revised periodically and the 5th edition is expected to be released in 2013. While there are many controversial proposed changes, one that has received relatively little attention in the mainstream media is particularly alarming in its potential implications for people with chronic illnesses, especially ones that are still scientifically poorly understood, like irritable bowel syndrome or commonly overlapping conditions like fibromyalgia, chronic fatigue syndrome and interstitial cystitis among others.

According to Suzy Chapman, a health advocate in the United Kingdom in her article, ?Somatic Symptom Disorder Could Capture Millions More Under Mental Health Diagnosis,? it is proposed that four separate and stricter diagnostic categories currently in the DSM-4 be revised to a single? umbrella diagnosis in the DSM-5 to be named ?Somatic Symptom Disorder.? As paraphrased in ?Mislabeling Medical Illness as Mental Disorder? by Allen Frances, MD in the U.S. magazine Psychology Today, Chapman writes in part:

?A person will meet the criteria for SSD by reporting just one bodily symptom that is distressing and/or disruptive to daily life and having just one of the following three reactions to it that persist for at least six months: 1) ?disproportionate? thoughts about the seriousness of their symptom(s); or 2) a high level of anxiety about their health; or, 3) devoting excessive time and energy to symptoms or health concerns.?

As noted by both Chapman and Dr. Frances, as well as Tiffany Taft, PsyD of Northwestern University and Oak Park Behavioral Medicine, whose psychotherapy practice specializes in IBS, inflammatory bowel disease and other chronic medical conditions, these criteria are extremely broad, very subjective according to the personal definitions and potential biases of an individual mental health professional, and could potentially be interpreted to encompass almost anyone with a disruptive chronic illness, even a relatively well-defined one such as cancer, diabetes, rheumatoid arthritis or life-threatening food allergy, at a given point in time. In many situations, the person may be expressing typical and understandable stress or grief related to chronic pain or other ongoing symptoms or risks.? In her own blog post on this subject, ?Moving In the Wrong Direction,? Dr. Taft also raises concern that normal emotional reactions of a parent or caregiver of a child with a chronic illness may be pathologized as well.

Specific to IBS and commonly overlapping or other complex functional syndromes, Dr. Frances quotes Chapman:

?The Work Group is not proposing to classify Chronic Fatigue Syndrome, Irritable Bowel Syndrome, and Fibromyalgia within the DSM-5 ?Somatic Symptom Disorders? section, but these patients and others with conditions like chronic Lyme disease, interstitial cystitis, Gulf War illness and chemical injury will now become particularly vulnerable to misdiagnosis with a DSM-5 mental health disorder. In the field trials, more than one in four of the irritable bowel and chronic widespread pain patients who comprised the ?functional somatic? study group were coded for ?Somatic Symptom Disorder.?

Dr Frances, who is listed in his article as the chair of the task force for the DSM-4, former chair of the Department of Psychiatry and current Professor Emeritus at Duke University School of Medicine, agrees that this new proposed diagnosis of Somatic Symptom Disorder may encourage a premature rush to label a person?s medical condition as ?all in the head.?

This disconcerting possibility is not unfounded. Historically, because IBS shows no overt biological markers that can be discerned by existing tests available to clinicians outside of highly specialized functional gastrointestinal research labs, it was long believed that there was nothing physically wrong with people with IBS. Ongoing research worldwide over the past 20-30 years has increasingly disproved that assertion, but widespread stereotypes and misconceptions remain.? Many people who have had IBS for some time can anecdotally recall at least one relative, acquaintance or health care professional who told them that IBS was ?all in the head? or similar dismissive comments, and even many people with IBS themselves continue to believe that IBS? is ?caused? by emotional stress rather than, more accurately, potentially worsened by stress depending on the situation and the individual. As IBS Impact wrote on this blog on October 9, 2011, a study conducted by UCLA in 2010 showed that compared to an international group of functional GI disorder experts, considerably more gastroenterologists, primary care physicians and nurse practitioners in local communities still support the outdated notion that IBS is a diagnosis to be given only when all other possible disorders are ruled out. In addition, Dr. Taft?s research interests focus on stigma in gastrointestinal disorders. She and her colleagues at Northwestern, in cooperation with Olafur Palsson, PsyD at the University of North Carolina Center for Functional GI and Motility Disorders, have found that study participants with IBS or inflammatory bowel disease both perceive stigma, but those with IBS perceive more. Dr. Taft?s blog post linked here includes links to the abstracts of her team?s published results for both groups.

It is hoped that there will be further revisions to this portion of the DSM-5 before the final version is released, but at this time, there is no clear information as to if this is planned. Regardless of the outcome, this situation underscores once again why it is important for those of us with chronic, poorly understood conditions like IBS, our loved ones, and the professionals who support us to make conscious efforts to begin or continue public awareness and advocacy about the realities of life with IBS , even if it requires stepping out of our initial comfort zones. If we do not speak out against this slippery slope, we risk even further invisibility and stigmatization in the future.

for the past couple of months, i been so forgetful plus my mind feel drained and in need of work out. it involves short-term memory and i forget what i was going to do. sometimes when i make food, i forget what i need to do. idk if its sign of depression because i experienced it off and on or my sleeping pattern contributing to it. i wake up at 11 am or 12 and go to bed at 2 am or 3 am. i get so distracted these days and i want my good memory back.

Gunman?s computers may be key in investigation

NEWTOWN, Conn. ? Alleged school shooter Adam Lanza reportedly occupied two bedrooms in his family's sprawling suburban home, one where he slept and another to stash his computer equipment.

For a young man who has been described as withdrawn from the outside world, Lanza's computer room is likely a gold mine for detectives, a veteran law enforcement source familiar with the investigation told Yahoo News.

"If he visited certain websites, they are going to glean whatever information they can from that and see what it means," said the source, who spoke on the condition of anonymity because he is not authorized to discuss the investigation publicly. "Does he have friends he communicates with online? Was there a fight with somebody?"

Police have already hinted that evidence inside the 4,000-square-foot home has been helpful in determining a possible motive for the rampage that claimed the lives of 20 children and six adult staffers at Sandy Hook Elementary.

Lanza shot and killed his mother, Nancy Lanza, at their home before driving to the school and commencing his deadly rampage, police say. Adam Lanza's body was later found inside the building, where police believe he took his own life. Results from the autopsies on the alleged gunman and his mother are still pending.

Lt. J. Paul Vance with the Connecticut State Police told reporters Sunday morning that investigators are scrutinizing the guns Lanza took to the school, including the semiautomatic military-style rifle the state's medical examiner said was used in the school killings.

"The weaponry involved, we are tracing historically all the way back to when they were on the workbench being assembled," Lt. Vance said.

However, he declined to discuss what else has been recovered from the Lanza home.

"Simply stated, we have a great deal of evidence that we're analyzing," Lt. Vance said. "The forensic part is an important part. That's not done yet."

While the gunman is thought to have acted alone, the law enforcement source said a deep dive into Lanza's computers could provide more clues.

"You don't know if this kid was put up to this by somebody else," the source said. "You don't know if there was a conspiracy of sorts. You don't know if there wasn't somebody who wasn't goading this kid on."

Family and friends say Lanza suffered from a personality disorder and that his mother, whom he killed just prior to the school shootings, struggled with her troubled son.

"Has he been seeing a child psychologist throughout his lifetime? Was he on medication?" the law enforcement source said. "These are a zillion logical who, what, whey, why, where questions that need to be answered. They need to be asked without any fear of any stigmatism ? and you can't be politically correct in asking those questions."

Nor should the public be shy about discussing whatever is learned about Lanza's life and what prompted him to act, forensic psychologist Kris Mohandie told CNN.

"The opportunity is nearly always there to discover and disrupt," he said.

Dr. Mohandie said warning signs can include self destructiveness, hopelessness, desperation, interest in other mass shooters and a dysfunctional interest in weaponry.

Police say the guns used in the rampage were apparently owned and registered to Lanza's mother.

Without knowing specifics about the Lanza household, Dr. Mohandie pleaded for greater care with firearms.

"If you've got individuals who are unstable and you know it, it's probably a good idea restrict their access to firearms within their own home," he said.

As signatories to the National Mortgage Settlement, we the undersigned state attorneys general write to urge you to pass legislation extending tax relief for citizens who have mortgage debt canceled or forgiven because of financial hardship or a decline in housing values. Such legislation is currently included in Section 112 of the Family and Business Tax Cut Certainty Act of 2012 (S. 3521), which was recently passed out of the Senate Finance Committee with bipartisan support. We strongly urge Congress to extend this critical tax exclusion, which expires on December 31, 2012, so that distressed homeowners are not stuck with an unexpected tax bill or deterred from participating in this historic settlement.

Las Vegas Real Estate Blog: Bank Foreclosures Start Again in Las ...

First, a little background:

Most large banks (Bank of America, Wells Fargo, etc.) are members of MERS, the Mortgage Electronic Registration System. This system allows a third party entity, like Recon Trust, to transfer notes between members of the system without recording the deed of trust into the new note holder?s name. So the name on the deed of trust and the name of the new note holder do not match since MERS was not required to record these documents. This made it difficult, if not impossible, for a homeowner facing foreclosure to actually find out who was foreclosing on them and try to work out an agreement. Many homeowners were foreclosed upon without ever being able to contact the existing note holder.
So in light of these national ?robo signing? scandals, Nevada was among 11 states that enacted legislation to protect homeowners and regulate illegal foreclosure activity. In Nevada, Assembly Bill 284 was passed which required a bank employee to sign an affidavit stating that to the employee?s personal knowledge all previous transfers of the note were correct before the bank could start the foreclosure process.
Since there is no bank employee that could possibly attest to previous transfers of a note, AB284 effectively stopped the Las Vegas bank foreclosure process in Nevada, and foreclosure filings dropped from around 6,000 per month to less than 1,000. (Not all banks were affected by AB284 since they were not members of the MERS system.)

In October of 2012, however, the Nevada Supreme Court ruled that banks using the MERS system would be able to foreclose on properties as long as the note holder and the name on the deed of trust matched prior to the foreclosure sale. ?This ruling will allow the third party entities used by MERS to record a deed of trust in the name of the current note holder and consummate the foreclosure process. Recording the deed of trust gives homeowners the opportunity to contact the note holder and work out a settlement, but doesn't allow them a "free ride" to stay in their homes indefinitely with no payment.


What this means for the Las Vegas real estate market:
The housing inventory has been at an extreme low for the past eight months, with bank owned foreclosures of Las Vegas single family homes accounting for only 10% of the active listings at any given time. Active short sales account for about 20% of the active single family listings and the rest are ?real? sellers. It?s a ?seller?s? market, prices have risen steadily since January, there are multiple offers on most listings within days of coming on the market, and buyers are finding it difficult to make a purchase unless they bid above appraised value. ??

Banks have started the foreclosure process again, and we do expect new inventory to begin?hitting the market after the first of the year, probably in or March or April. However, the banks do still have very stringent hoops to jump through, and there is no huge wave expected all at one time. Still, there should be more?listings on the market, allowing frustrated homebuyers the opportunity to finally get an offer accepted. Hopefully we will get back to a more balanced market that remains steady for the next few years. A balanced market is better for both buyers and sellers and the local economy.

Strong Social Support Seems to Boost Breast Cancer Survival ...

THURSDAY Nov. 15, 2012 -- The quality of a breast cancer patient's personal relationships may be as important as the size of her social networks in predicting survival, researchers report.

The new study included more than 2,200 women who were diagnosed with early-stage, invasive breast cancer between 1997 and 2000. After an average of 11 years follow-up, 215 of the women had died from breast cancer and 410 had died from all causes.

At the start of the study, the women provided information about their personal relationships, which were characterized as socially isolated (small social network), moderately integrated or socially integrated (large social network).

A social network includes spouses or partners, female relatives, friends, religious and social connections, and links to the community through volunteering.

Women who were socially isolated were 34 percent more likely to die from breast cancer or other causes than women who were socially integrated, the Kaiser Permanente researchers found.

The investigators also discovered that levels of support within personal relationships were important risk factors for breast cancer death.

"Women with small networks and high levels of support were not at greater risk than those with large networks, but those with small networks and low levels of support were," study lead author Candyce Kroenke, a research scientist with the Kaiser Permanente Northern California Division of Research, said in a Kaiser news release.

Women with small social networks and low levels of support were 61 percent more likely to die from breast cancer and other causes than those with small social networks and greater levels of support, the study authors pointed out in the news release.

"We also found that when family relationships were less supportive, community and religious ties were critical to survival. This suggests that both the quality of relationships, rather than just the size of the network, matters to survival, and that community relationships matter when relationships with friends and family are less supportive," Kroenke added.

The study was published in the current issue of the journal Breast Cancer Research and Treatment.

While the study found an association between patients' social support networks and survival rates, it did not prove a cause-and-effect relationship.

World Bank, ADB eye clearing Myanmar's overdue debt in Jan ...

TOKYO, Oct. 11, Kyodo

The Asian Development Bank and the World Bank, Myanmar's second and third largest creditors, expressed their intention Thursday to clear the Southeast Asian country's overdue debts to them in January, Japanese officials said.

The envisioned move, unveiled at a meeting of officials from Myanmar and its main creditors in Tokyo, will pave the way for the international community to start providing full-fledged development assistance to the long-isolated country.

Japan, Myanmar's largest creditor, said it will implement what it calls an "arrears clearance operation" next January and plans to resume yen-denominated concessional loans to Myanmar at the earliest time possible next year.

Following the meeting, which was hosted by Japan on the sidelines of the annual meetings of the World Bank and the International Monetary Fund, the Japanese government began arranging tens of billions of yen in such loans to Myanmar next year, government sources said.

During the meeting, participants agreed on the need to unite behind Myanmar's reform efforts in order to facilitate the country's reengagement with the international community, according to Japanese officials.

"Addressing Myanmar's debt issue in a comprehensive way is important for Myanmar to engage in the international community," the country's finance and revenue minister Win Shein told the meeting, expressing his appreciation to Japan and other creditors.

The meeting was called mainly to chart a clear path toward settling the roughly $900 million in overdue debts to the ADB and the World Bank. The settlement has been eagerly awaited by Japan, which has already agreed to resolve Myanmar's roughly 500 billion yen debt to it but has yet to implement the debt relief.

"It is high time that the international community united in its efforts to underpin reform efforts by Myanmar and reintegrate Myanmar as one of its members," Japanese Finance Minister Koriki Jojima told the meeting, noting that a series of reforms are under way in the country.

If resumed as planned, yen-denominated concessional loans to Myanmar would be Japan's first such lending to the country in 26 years. Myanmar, rich in natural gas and other resources, has been seen as a promising new investment destination for Japanese firms.

Officials from the World Bank and the ADB told the meeting that they were making necessary preparations to help Myanmar clear its debt arrears in January, Japanese officials said.

Under the envisioned clearance, Myanmar would repay its delinquent loans to the international lenders by borrowing bridge loans from the Japan Bank for International Cooperation. The lenders would then make fresh low-interest loans to the country.

Myanmar owes about $500 million to the ADB and $400 million to the World Bank. Countries that have borrowed from these international lenders must clear their debt arrears before they can ask for new financial aid.

Participants in the meeting also agreed that the two international lenders' full engagement is essential for Myanmar's economic reform and that active engagement by the IMF is also indispensible in reforming the country's macroeconomic management, the Japanese officials said.

Japan announced in April that, of the approximately 500 billion yen owed by Myanmar as a result of its delayed repayment of past loans, Tokyo will forgive about 300 billion yen, becoming the first creditor country to make such a move.

Myanmar is expected to repay the remainder by taking bridge loans from Japanese commercial banks, while Japan is to resume low-interest development loans to help the country implement reforms.

Jojima, Japan's financial chief, said yen-denominated concessional loans would be extended to finance projects involving electricity, road construction, rural development and port facility development.

During the meeting, Myanmar, which has also borrowed from such countries as Germany, Denmark and France, was invited to a meeting in Paris in January to negotiate a multilateral agreement with the so-called "Paris Club" creditor countries.

Officials from 26 countries and five international organizations attended the meeting. Chinese officials, who were scheduled to attend the event, did not show up, the Japanese officials said.

Since President Thein Sein established a reform-minded government in March last year, Myanmar has begun opening up to the world after years of military rule, making a transition to democratic governance.

The ADB forecasts Myanmar's economy will grow over 6 percent annually in 2012 and 2013. The prospect has raised hopes that the resumption of international assistance to Myanmar will help the Southeast Asian economy grow further.

We did perform one more test, however, and that was to shake up a can and just let it sit. After about two minutes, we opened it up with no problem. So, while fancy tricks may help keep you a little drier, the best solution is to just have a little patience?those bubbles should go away on their own after a few minutes.

Animals suspected in spread of new virus

LONDON (AP) ? Britain's Health Protection Agency has published an early genetic sequence of the new respiratory virus related to SARS that shows it is most closely linked to bat viruses, and scientists say camels, sheep or goats might end up being implicated too.

So far, there are no signs the virus will be as deadly as SARS, or severe acute respiratory syndrome, which killed hundreds of people, mostly in Asia, in a 2003 global outbreak.

In Geneva, WHO spokesman Glenn Thomas told reporters Friday that so far the signs are that the virus is "not easily transmitted from person to person" ? but analyses are ongoing.

Global health officials suspect two victims from the Middle East may have caught it from animals.

"It's a logical possibility to consider any animals present in the region in large numbers," said Ralph Baric, a coronavirus expert at the University of North Carolina at Chapel Hill. "Biologists now need to go into the area and take samples from any animals they can get their hands on, including camels and goats," he said. Baric said it was crucial to find out how widespread the virus is in animals and what kind of contact might be risky for people.

Baric suggested bats might be spreading the virus directly to humans since the two confirmed infections happened months apart. "If there was an established transmission pattern from other animals, we probably would have seen a lot more cases," he said.

The World Health Organization said it is considering the possibility the new coronavirus sickened humans after direct contact with animals. The agency is now working with experts in the Middle East to figure out how the two confirmed cases got infected but could not share details until the investigation was finished.

One patient was a Saudi Arabian man who died several months ago while the other is a Qatari national who traveled to Saudi Arabia before falling ill and is currently in critical but stable condition in a London hospital.

Earlier this week, WHO issued a global alert asking doctors to be on guard for any potential cases of the new respiratory virus, which also causes kidney failure.

Saudi officials have already warned that next month's annual Muslim Hajj pilgrimage, which brings millions to Saudi Arabia from all around the world, could allow the virus to spread. As a precautionary measure, they are advising pilgrims to keep their hands clean and wear masks in crowded places.

Experts said knowing where a virus comes from provides clues on how to stop it.

"This means we could prevent the fire before it starts instead of rushing towards it with fire trucks and water hoses afterwards," said Michael Osterholm, an infectious diseases expert at the University of Minnesota.

Osterholm said it was possible bats had simply passed on the virus from other animals and that there could be a complicated transmission chain that ultimately ended in humans.

Viruses reproduce as they infect animals and people, giving them more chances to evolve into a deadlier version.

"We don't know enough about coronaviruses to predict which mutations might make them more lethal or transmissible," Osterholm said. "But you don't want to tempt genetic fate with microbes because you're bound to lose most times."

Hm I prefer the characters have some kind of detail to them, but you don't have to go all out for this RP. Normally my character sheets a rather long, but I chose to go with the basics for this one.

(AP) ? Paula Creamer took one look at the wind and rain battering the Royal Liverpool links and liked what she saw.

Creamer will play alongside defending champion Yani Tseng of Taiwan and Ai Miyazato of Japan for the first two rounds of the Women's British Open, the season's fourth and final major starting Thursday at Hoylake.

"This course sets up incredibly well for me," Creamer said. "I hope it stays this windy and this hard. I like that. I truly like the challenge."

Creamer experienced the challenge of longest playoff between two players in LPGA Tour history, losing to Jiyai Shin on the ninth extra hole Monday at the Kingsmill Championship.

With heavy wind and rain affecting Wednesday's practice and more bad weather forecast for the next few days, the 36th staging of the tournament could become a war of attrition for the world's best women golfers.

The conditions were much different when the American played Hoylake for the first time on a private visit this summer.

"When I played, I was in shorts and short-sleeved shirt," she said. "I actually played it twice. The first time there was no wind and the second day it was like today, when it died down a little bit.

"Wind like this, conditions like this, a lot of it you can't control. But I think it was smart coming to have a look when we did. I know where to avoid and where not to go."

Creamer is a global ambassador for tournament sponsor Ricoh, which announced an extension of its commitment to the event for another three years until 2016

Tseng, winner at Carnoustie last year and Royal Birkdale in 2010, was full of praise for the Royal Liverpool Golf Club. It has hosted 11 men's British Opens and many top amateur and professional events in its history, but is staging the women's championship for the first time.

"I just love the course, the British Open with its history and tradition," said Tseng, who will be bidding for her sixth major championship since her first victory as a 19-year-old at the 2008 LPGA Championship. "I think you have to have so much imagination out there. You really need to work the ball.

"I've struggled a little in the last couple of months, but it's a good time to be back here and I think it's my turn to start playing well again. I love playing with Ai and Paula. It's a good draw and I'm very excited. It should be fun."

Miyazato, with eight top 10 finishes on the LPGA Tour this year, is looking forward to the challenge.

"It's really nice to be here. I just love the atmosphere of this tournament," Miyazato said. "My short game is pretty solid this year and that's why I think I've been playing really well so far."

The trio will start Thursday from the 10th tee. Three other former major winners ? Sweden's Anna Nordqvist and South Koreans Inbee Park and Jiyai Shin ? start at the same time from the first tee.

The winners of this year's first three majors ? China's Shanshan Feng and South Koreans Sun Young Yoo and Na Yeon Choi ? are also in the field.

Michelle Wie, Christie Kerr, Brittany Lincicome, Lexi Thompson, Morgan Pressel, Natalie Gulbis and Brittany Lang are among the 34-strong American contingent in the field of 144.

Fifteen-year-old New Zealand amateur sensation Lydia Ko, winner of the Canadian Open last month, tees off first from the 10th paired with Lexi Thompson and Japan's Kaori Ohe. The South Korean-born Ko became the youngest winner in LPGA Tour history and only the fifth amateur champion.

The European challengers include 20-year-old Scot Carly Booth, leading the tour order of merit with two wins this season; Germany's Caroline Masson, who led going into the final round at Carnoustie last year; and Sweden's Caroline Hedwall, winner in Austria 10 days ago.

Catriona Matthew, the last British winner at Royal Lytham in 2009, is one of the home favorites along with European Tour veteran Laura Davies, who won the title in 1986.

American Numa Gulyanamitta withdrew Wednesday and was replaced by first alternate Louise Larsson from Sweden.

Brits ready to stake their preorder claim this Friday might have a harder time deciding between the myriad of networks offering the device. While all of the major UK carriers will be ready to offer you the iPhone 5, that LTE radio will only be working on the newly-christened EE from the start, with Three likely to use that purchased spectrum (after getting the okay from Ofcom) to similar effect in the near-future. Unfortunately for Vodafone and O2, the forthcoming Ofcom spectrum auction is gearing up to sell off the 800MHz (Band 20) and 2.6GHz (Band 7) frequencies -- neither of which are found on Apple's multiple iPhone 5 models, and the latter being a bigger problem in Europe and Asia, where LTE networks already make use of the 2.6GHz frequency. However, Apple have followed up an initial phone launch with additional network-specific models before -- so there's a slim chance we could see another model at a later date. All the UK carriers are keeping their contract prices a tightly-guarded secret at the moment, but we'll update as soon as we hear more. Meanwhile, those on that little European isle can expect to pay £529 for the entry-level iPhone 5 from the source when pre-orders start this Friday.

Update: Pocket-lint's been told that those looking to grab the EE iteration will need to initially register with either Orange or T-Mobile, with your service bizarrely migrating across on a later, as-yet unconfirmed, date. This is because its new 4G network won't be ready when the iPhone 5 first hits shops on September 21st. Before that, you'll have to make do with HSPA+ speeds.
